Smiles by Schwam Extreme Smile Makeover Contest Rules: Contest is limited to residents of Coos and Curry Counties. Applicants residing outside these areas will not be considered. Contestants will submit a 500-word essay stating why they believe they are the best candidate for the Extreme Smile Makeover. Essays should be detailed in description and MUST include a current headshot photo as well as a close up of the contestant’s teeth and smile. Five Finalists will be chosen and will each receive a complete exam, a full mouth set of x-rays and a professional cleaning (valued at $245.00). Condition of oral health MUST be natural, hereditary or the result of accident or injury. Contestants who abused their health through drug and/or alcohol use or through negligence will be disqualified. Winner of the contest MUST be flexible and available for treatment when scheduled. Depending on the course of restoration, there may be several visits required. Finalists may be subject to drug testing. All contestants MUST be 21 years of age or older. Winner of the contest MUST be accessible to the press and will consent to publication of name and before and after photos. Winner of the contest will receive a full mouth restoration valued at up to $50,000. Entries must be received by April 15, 2005. Winner will be chosen on May 10, 2005 and notified by phone.
